This guide is a simple, writeable PDF that sites can customize with sections including sexual health services, mental health services, afterschool programs, school-based health, or nutrition services. Health centers and youth-serving community-based organizations alike can populate the template with their community’s resources and either make it available for adolescents, or use it for their own internal use.
