Program Impact Reports 
This set of documents was prepared to help local health departments and MALPH members to tell the story of how their programs and services impact the health of the community.  There is a “statewide” set which describes the impact of all local health departments who do this service across Michigan, and there is a “local” set, which may be edited and tailored to each local health department.  Barry-Eaton District Health Department (BEDHD) was the author of these documents as part of an MDHHS-funded Cross-Jurisdictional Sharing grant in 2018.  As such, the “local” documents refer to BEDHD as an example.  These may be used in a variety of ways – we look forward to seeing how you use them!
