The Health Fund makes grant investments through the initiative to support community-driven, locally focused projects that incorporate at least one of three priority categories:Addressservice delivery environmentbarriers to improve access to equitable pre-conceptual and perinatal health care through improved integration of medical, behavioral, public health, and social services.Increase access toprotective and clinical interventionsincluding improved screening before, during, and after pregnancy for health risks, as well as referral and connection to treatment or services.Address targetedpolicies, practices, and proceduresat the local or practice level that impact or prohibit the best outcomes for birthing parents and infants.
