Return registration form with check payable to “UPEHA”,  to Jennifer Farnworth, c/o Marquette County Health Department, 184 US 41 E. Negaunee, MI 49866. Please provide a separate registration form for single day registrants.

Don't forget to make your hotel reservations by January 28, 2019, and make sure to mention the UPEHA conference room block to receive the discounted room rate of $63.70/per room (plus tax).
