The Michigan Public Health Week Partnership* invites you to join them in celebrating Public Health Week in Michigan by nominating someone for the Hometown Health Hero award. The Hometown Health Hero award is presented to individuals and/or organizations that have made significant and measurable contributions to preserve and/or improve their community’s health.

The Michigan Public Health Week Partnership has adopted the theme for National Public Health Week: “Centering and Celebrating Cultures in Health.” Our cultures have always shaped our health. We learn from the communities we're born in and that we build together. We look to community leaders as our health champions. We celebrate the unique and joyful ways different cultures focus on health. We can learn from each other, with humility and openness. Understanding the cultures in our communities helps achieve health equity. The daily themes for National Public Health Week can provide additional context. The daily themes are: Community (Monday), Violence Prevention (Tuesday), Reproductive and Sexual Health (Wednesday), Mental Health (Thursday), Rural Health (Friday), Accessibility (Saturday), Food and Nutrition (Sunday).

Community leaders, organizations, and individuals are doing great things to improve the health of their communities and to increase equity in health. The Partnership wants to recognize those accomplishments, but we need your help. You know your community best. What health improvements are happening in your community that we can potentially recognize with this award? To help you answer that question, look at those who have received this award in the past. The only way for someone to receive the award is to be nominated. Submit the nomination form below by email or fax no later than February 10. Awardees are selected from nominations received by the deadline.

Awards will be presented at 11:30 a.m., Tuesday, April 11, in the new Heritage Hall of the State Capitol Building.
