Knowing the details of clients’ social determinants of health (SDoH) can make a significant difference in the quality of healthcare they receive and often those who provide care only have a fraction of those details. There are many challenges to collecting SDoH data, but the Michigan Health Information Network (MiHIN) aims to partner with stakeholders statewide to make data collection easier.
In this webinar the following objectives will be covered:
- Explore MiHIN’s current projects and roadmap to advance statewide SDoH data exchange.
- Discuss ways to leverage data to provide holistic care.
- Provide actionable steps for a variety of stakeholders to support statewide SDoH data exchange.
