Note: 

Innovative dementia programs can be translated and expanded to provide benefits to wider populations and as a vehicle for sustainability. During the COVID-19 pandemic, Hospital2Home (H2H), a highly successful dementia care transitions program, was translated to provide discharge relief to over taxed hospitals and to prevent readmissions. A data driven approach resulted in a successful collaboration with the County and expansion of H2H to other difficult to discharge complex and vulnerable populations. This webinar will address how data can create a pathway for program expansion and sustainability.

Goals/Learning Objectives:
1. Participants in this webinar will be provided with an overview about the Hospital2Home program and how it can be used for people living with dementia and their caregivers as well as people with other medical conditions, including COVID-19.
2. Participants in this webinar will learn how the Hospital2Home program was expanded during the height of the COVID-19 pandemic.
3. Participants in this webinar will learn about the importance of gathering and analyzing data about their programs to aid in determining aspects of the program to sustain.
