Under the supervision of the Director/Supervisor of Personal and Community Health Services, the PHN II addresses community needs by providing comprehensive nursing skills and services including assessment, diagnosis, teaching, counseling, prevention, and referral services to individuals, families, and groups to promote health throughout the county. Coordinates and participates in the operation of clinics and programs to promote health and wellness to clients in the county. Maintains patient records and program activity records and prepares required reports. The PHN II solves problems with considerable autonomy in working with individuals, families, and groups from the most vulnerable in the community to other professional groups. Prepares and maintains patient records, program activity records, required reports, program policies, and accreditation material. The PHN II may also be expected to provide leadership for specific programs.
