Join the MDHHS Health Access and Innovation Division for a webinar exploring the evolution of Michigan’s Roadmap to Healthy Communities, the MDHHS social determinants of health strategy, and what comes next for this work.

Participants will learn about progress made through the Roadmap, the transition to the Health Access and Innovation Division and how MDHHS is building on partnerships and lessons from Michigan Hubs, Community Information Exchange, community health worker initiatives and other efforts. The webinar will also provide updates on the Connected Communities Task Force and opportunities to strengthen access, care coordination and community-driven solutions across Michigan.
